This form is to be used by anyone who wishes to report discrimination. It is not to be used for other types of complaints, raising safeguarding issues or to make general enquiries.

You can choose whether to report discrimination anonymously or alongside your name and contact details. This makes no difference to the seriousness with which the club will deal with your report. However, without specific information it may be difficult for us to investigate your complaint.

Please provide as much information as possible. The more detail that we have, the more effectively we can deal with the issues raised.
